Our free 360 Degree Feedback Form template makes it easy to incorporate peer review feedback into your own organization’s HR employee performance management processes. This template includes the related Employee List form, which populates the data in the Employee Name and Email fields.

The form’s authentication settings require the manager to log in using their Cognito Forms username and password. To initiate the workflow, the manager requests performance reviews from two of the employee’s peers. Once the manager submits the form, a new entry is automatically created in the Reviewer 360 Degree Feedback Form. This form allows the selected peers to evaluate the employee’s performance.

After both peers complete their reviews, the manager receives a notification indicating that the 360-degree review process is complete. You can customize this template to align with your peer feedback process by adjusting the number of peers involved in the review.
