How It Works
- Log in to Zapier
- Access your Zapier dashboard
- Create a new Zap
- Click “Make a Zap”
- Set Cognito Forms as the trigger
- Choose “Cognito Forms” → “New Entry”
- Add Slack as the action
- Select “Send Channel Message” or “Send Direct Message” in Slack
- Connect your Slack account and pick the channel or user
- Map entry fields to Slack parameters
- Link form fields to message text, attachments, or blocks
- Test the Zap
- Submit a test entry and verify the Slack alert
- Activate the Zap
- Turn on the Zap to enable continuous notifications

- Add “Watch New Entries” module for Cognito Forms
- Use the Create a Webhook / Choose a hook
