Use our free CAHPS Clinician & Group Adult Survey to collect feedback from your patients about their experiences with your providers. All required questions and content are built in using the publicly available survey content and adapted to an online format. Using our conditional logic features, questions that patients would skip on a paper survey will be hidden and skipped on the online form.

An internal section is included for your staff to view internal codes regarding the survey’s completion status. Using CAHPS survey completion guidelines, calculation fields have been added to calculate how many reportable and key items were completed by the patient. Using these calculation fields, another field will pop up for internal staff to see if the survey is complete, partially complete, or incomplete.
