Reviewer Feedback Form with Connected Employee List

Our free Reviewer Feedback Form template makes it easy to incorporate peer review feedback into your own organization’s HR employee performance management processes. This template includes the related Employee List form, which populates the data in the Employee Name and Email fields.

First, the form’s authentication settings require the manager to log in with their Cognito Forms username and password. The manager begins the workflow by requesting two of an employee’s peers to complete a performance review. Each individual is asked to answer a question that rates the employee’s performance, as well as to provide their signature on the form. Once both peers have filled out their reviews, the manager is notified that the review is complete. Customize this template to fit your own peer feedback processes by adding any number of peers to review the employee.
