Rating Scales Are Now Available on Cognito Forms

The new Rating Scale feature in Cognito Forms makes creating online surveys a breeze! Rating scales, also known as Likert scales, provide a matrix of choices to allow respondents to rank statements or questions based on their level of agreement or disagreement. In Cognito Forms, these scales are highly customizable, providing a unique experience for all users. Default response settings, answer requirement, and unlimited questions are just a few of the ways you can customize your form.

For your convenience, the team has put together predefined scales with the option to add a custom scale of your choice:

  1. Satisfied/Unsatisfied
  2. Agree/Disagree
  3. Will/Won’t
  4. Good/Poor
  5. Scale of 1 to 5
  6. Custom

Once a scale is chosen, you can easily select a default value from the advanced settings section. The default value is the value automatically selected before a respondent provides an answer – i.e. a response of Neutral.

Beyond this, you can customize to your heart’s content by:

Rating Scale options

  • Editing predefined questions in templates
  • Changing the rating style of the respondent’s answers
  • Adding or deleting questions as needed
  • Allowing an N/A response that can have a customized label
